All coach concerns will be addressed by completing, signing and dating the top portion of the form below and emailing it to the Fast Break Club President. Any anonymous complaints will not warrant intervention, and be treated as hearsay. With the exception of physical abuse, The Fast Break Club encourages each player(s) and/or parent(s), after a 48 hour waiting period (NOT the day of a game) to first address concerns directly with the coach. Allegations of physical abuse should be brought to the attention of the Fast Break Club IMMEDIATELY!
